Adjusting To Your New Life After LAP-BAND®

LAP-BAND® surgery can be a very effective way to lose weight over time, providing you are committed to doing the work that comes with your new Band.

Once the Band is in place, you will need to be extremely conscious of how you treat your body, and make sure you are doing everything possible to optimize health and encourage safe, gradual change.

Here is some general information on various aspects of life after the procedure.

Recovery: Most patients will be discharged within 2 to 4 hours of surgery. Typically, you can expect to return to work within one week, and physical exercise including lifting can be resumed around the 3rd to 4th week.

Physical fitness: After you are fully healed from the procedure, you will be able to do the physical activities you enjoyed beforehand, plus any new types of exercise that will help you burn calories and reach a comfortable fitness level.

Return visits: You will be asked to return to the surgeon’s office 1 month after your surgery for your 1st fill. Follow-up visits depend on your needs. Once your “sweet spot” is determined, you will need adjustments once or twice a year.

Results: Remember that gradual weight loss is the goal. Most patients experience a loss of 1-2 pounds per week.

Fullness: The LAP-BAND® helps you feel full by reducing the capacity of your stomach and increasing the time it takes food to get leave the pouch and get through the Band.

Supplements: Patients who do not get enough protein or nutrients from meals alone will need protein supplements and daily multi-vitamins. Shakes are a great way to provide the body with the recommended daily intake of protein.

Diet and nutrition: Since you will be consuming smaller quantities, you’ll want to choose foods that are rich in vitamins, nutrients and antioxidants. A high-protein, low carbohydrate mix should be your aim for all meals and snacks. Chew your food slowly and limit your serving sizes. When eating out, order smaller portioned meals or just appetizers, or ask your server to box up half of your meal before it is delivered. Alcohol is discouraged and should be used rarely if at all due to its high calorie content and the fact that it goes through the Band without giving you a full feeling. Preparing Your Body for LAP-BAND® Surgery

When considering LAP-BAND® surgery as a weight-loss solution, there are some things you should know about how to prepare your body for the procedure. Since the Band is designed to be a permanent fixture (although it is removable), you will need to adjust your diet to get your body in tune for surgery, maximize the effects of the Band, and enjoy long-term success.

Here are a few guidelines from Dr. Gerald Kirshenbaum, Director and surgeon at Lap Band Rockies in Denver, Colorado.

Foods to Avoid

For the 10 to 14 days, depending on your BMI, before surgery (known as the pre-operative period), you will be instructed to eliminate the grain group completely. This means no complex carbohydrates, including pasta, rice, potatoes, bread, whole-grain cereal, pastries, or anything made from white flour, including pretzels, chips, tacos, tortillas, cupcakes, cookies, etc.

Simple sugars are allowed, so you may use sugar with your coffee, yogurt, fruit and veggies, and other such foods. You should try to consume a range of 70 – 120 grams of protein per day (chicken, fish, protein supplements, etc.), depending on your body type. Your surgeon will recommend a specific calorie range that you should abide by during your pre-operative period and provide a list of acceptable foods and beverages.

It is important to stick to your pre-operative diet and to avoid splurging in order to optimize your surgical results. These pre-operative modifications are also needed to shrink the liver to create the proper condition to permit the surgery.

Understanding the Band Concept

Before you make the necessary changes in your eating habits that will prepare your body for the Band, you should understand how the Band works. The Band’s job is to help you feel satisfied with a smaller volume of food. You can train your mind to think of food as fuel to be used only when you feel hungry to avoid habitual snacking or emotional eating. After the Band is in place, you will get used to following a high protein/low carbohydrate formula, so cutting out the grain group before surgery should be good practice. Also after surgery, you will learn to eat slower, take smaller bites, and chew more thoroughly, all actions that make for a very controlled and effective way to eat.

Long-term Meal Planning After LAP-BAND®

The first four weeks after LAP-BAND® surgery require a very tailored approach to eating, as we discussed in our last blog post. After the initial familiarization period, it is time to think about your long-term eating habits. Dr. Kirshenbaum and his team will discuss their optimal recommendations for a healthy diet during your orientation process and help you develop a long-term plan you can use to provide structure for your new lifestyle.

After you’ve lived with the Band for a few months, you will have learned a lot about what your body needs in terms of liquid and solid nutrition. As you slowly adjust to the Band, you will create a personal routine for what you can eat and how often. Here are some general guidelines on meal planning, snacking and supplements to help you get the most from your Band while supplying your body with the necessary nutrients.

Eat slowly.

  • Allow 20-30 minutes to eat a meal.
  • Take small bites of food.
  • Protein will give you a full feeling for a longer period of time.
  • You can make the band more efficient by and holding off on drinking for 30 minutes after eating.

Stop eating at the first feeling of fullness.

  • Eat protein foods first.
  • Pre-portion what you anticipate you will eat at a sitting to avoid having food left over in excess of your capacity.
  • Eating your meals on smaller plates and in smaller bowls will help you feel more emotionally satisfied.

Eat only when you are hungry.

  • Selecting healthy foods from the 4 food groups is important because the total amount of food you are eating is reduced.
  • Frequent eating or snacking throughout the day will keep you from losing weight or may even cause weight gain.
    • If you get hungry between meals, try a protein bar or shake
  • Avoid all high-calorie drinks, soda, beer, and other alcoholic beverages.
    • Drinking of high calorie beverages throughout the day will keep you from losing weight or may even cause weight gain.
  • Use protein supplements between meals.
    • Sip a high protein drink of your choice between meals.
  • Sip water or a calorie-free, non-carbonated beverage at all other times.
    • Try to drink 60 ounces of water a day.
  • Avoid carbonated liquids because they can stretch the pouch creating discomfort.

Remember:

  • Take a multivitamin and a calcium supplement of 1000 mg of calcium.

Guidelines For the Mature LAP-BAND®

With the LAP-BAND® procedure comes the responsibility to care for your body the right way, to both reach a healthy weight and ensure you are getting the proper nutrition you need for optimal health.

Many people who are considering LAP-BAND® want to know, “What is life like after the procedure?” We recommend you visit Lap Band Rockies’ previous blog

Recommendations for Healthy Eating to get an idea of your post-surgery nutrition needs.

Also, you will be encouraged to follow the guidelines below to maximize your results and reach your personal ideal health status.

Eat only if you are hungry

· Avoid opportunity eating, i.e. just because food is there

· Not based on the time of day (lunchtime, for example)

· Not because family or friends are eating

· Card requesting the courtesy of child/senior portions

Make good dietary choices (nutritious foods)

· Proteins first, veggies next, then fruits

· Solid foods – avoid soft mushy foods or liquid calories

· Avoid simple sugars

· Eat fruit rather than juices

· Meds: chewable, liquid and capsules (the kind that separate and the powder falls out)

· Pills larger than a peanut must be crushed to avoid injury to the stomach lining within the band

Avoid useless calories

· Useless calories are those which slip through the Band and won’t give you a satisfied feeling, i.e. alcohol, chocolate, ice cream, candy

Stop when you feel full

· Listen to the signs of fullness: distension, hiccups, or burping of food

· Take small bites and chew well

· A meal should last 20 – 30 minutes (longer is considered grazing)

Exercise

· 150 minutes minimum per week (30 minutes per day x 5). Do not be discouraged by weight gain after you start working out as muscle weighs more than fat.

Adjustable Gastric Band Vs. Other Weight Loss Procedures

Putting on weight is easy, especially since eating right and finding time for exercise can be difficult in today’s busy, demanding world. Excess calories and high-fat foods can catch up to you, leading to obesity and weight-related conditions like diabetes, heart disease, and sleep apnea. Thankfully, severely overweight individuals have more options than ever before for surgical weight loss. Here, we discuss three weight loss surgeries and some pros and cons.

Gastric Bypass

Gastric Bypass and Roux-en-Y Gastric Bypass is intended to help patients lose weight by manipulating the digestive system to absorb fewer nutrients and calories. The Gastric Bypass surgery involves cutting and stapling to reduce the size of the stomach, combined with a move that re-routes a portion of the intestine to the stomach pouch so it “bypasses” most of the stomach and part of the intestines. Gastric Bypass is not an adjustable surgery. Gastric Bypass creates malabsorption by the design of the operation. There may be an advantage for this operation for patients with diabetes. There are many post-operative complications including leakage because the gastrointestinal (GI) tract has been divided and rerouted.

Gastric Sleeve

Gastric Sleeve is a procedure that promotes weight loss by significantly reducing a patient’s stomach size. A stapling technique is used to create a very small stomach section, and then the larger stomach portion is completely removed. This permanent procedure does not directly alter the digestive process, but creates an environment where patients can only consume small amounts of food at a time. The risk of leakage is inherent in the procedure because the GI tract is violated. Gastric Sleeve is still considered experimental by many healthcare professionals and insurance companies, and the American Society for Metabolic & Bariatric Surgery does not recognize or endorse this procedure as a safe, effective weight loss option.

Adjustable Gastric Band

The Adjustable Gastric Band (LAP-BAND® or REALIZE® Band) is a minimally invasive surgery that produces steady, healthy weight loss by reducing the stomach capacity, so patients feel satisfied with a reduced volume of food. The Band also helps patients feel fuller longer by slowing the stomach’s emptying process. Healthy nutrition and a fitness program work in conjunction with the Band to induce weight loss.

How much weight will I lose, and when will I see results?

What can I expect during the recovery period?

These adjustments are done in the office. During an adjustment, the surgeon inserts a fine needle through the skin into the access port to add or remove saline.
